Lesiones de base craneal media central

Lesiones de base craneal posterior central-superior (regiones clival y petroclival)

Se puede combinar con abordaje fronto-témporo-zigomático.

Suboccipital retrosigmoideo.

Las lesiones de la región petrosa y clival pueden intervenirse quirúrgicamente con la combinación del abordaje suprainfratentorial presigmoideo.

El abordaje presigmoideo retrolaberíntico es el que ha tenido mayor aceptación debido a la amplitud de exposición sumado a la baja morbilidad para acceder a la región petroclival.

Una mastoidectomia minima ofrece el mismo angulo de exposición y similar cercanía a la región petroclival en comparación a la variante retrolaberíntica pero sin la morbilidad que significa exponer las estructuras neurovasculares y otológicas escondidas en la profundidad del hueso temporal.

Morrison y King fueron los primeros en reportar un abordaje presigmoideo translaberíntico y luego Hakuba propuso la vía presigmoidea retrolaberíntica. Hashi , Al-Mefty y Sammi agregaron variantes a estos abordajes y también a la vía translaberíntica y transcoclear ideada por House en 1964.
